STARTING THE PROCESS

First, fill out the following following on-line form:

- Driver Evaluation Request Form -

Next, print out and return the following two additional forms:

 (This can be done via fax or / and provided at the time of service)

- Release of Information -

- Client Contract -

Finally, contact your doctor and request a referral/prescription for a 'Drivers Evaluation'.  To simplify the process you can also print out our pre-printed prescription form and provide it to the doctor for their convenience.

* * * Once the MD Prescription & Driver Evaluation Request are received you will be contacted to arrange an appointment date & time.
